Glaucoma Care in Grand Cayman

Dr Behrad Samadi MD FEBO is an ophthalmic surgeon at Dr. Foley's Eye Clinic, 107 Claude Hill Road, George Town, specialising in cataract and glaucoma. He trained at Karolinska and Moorfields and looks after glaucoma patients from first diagnosis through long-term care, including laser and minimally invasive glaucoma surgery (MIGS).

What is glaucoma?

Glaucoma is a group of eye conditions that damage the optic nerve, the cable that carries visual information from the eye to the brain. It is often linked to raised pressure inside the eye, although it can also occur at normal pressure. In its most common form it develops slowly and without pain, and side vision is usually affected first, so many people do not notice any change until the condition is advanced. Vision lost to glaucoma cannot be restored, which is why early detection and steady treatment matter.

Diagnosis

A glaucoma assessment is painless and usually takes place in a single visit. It may include:

  • measuring the pressure inside the eye

  • examining the optic nerve

  • a visual field test to check your side vision

  • OCT scanning, which measures the nerve fibre layer at the back of the eye

  • assessing the drainage angle of the eye and corneal thickness

These results help Dr Samadi confirm whether glaucoma is present, what type it is and how best to manage it.

Treatment

Treatment aims to lower eye pressure and protect the remaining vision. The right approach depends on the type and stage of glaucoma, your other eye conditions and your preferences.

Eye drops

Daily pressure-lowering drops are often the first step. They work well for many people when used regularly. If drops cause side effects or are difficult to keep up with, other options are available.

Laser treatment

Selective laser trabeculoplasty (SLT) is a quick clinic procedure that improves drainage of fluid from the eye. It can be used as a first treatment or alongside drops. For certain types of glaucoma, a laser iridotomy may be recommended to open up a narrow drainage angle.

MIGS and surgery

Minimally invasive glaucoma surgery (MIGS) uses very small devices or incisions to improve the eye's natural drainage. It is often combined with cataract surgery, so both conditions can be treated in one procedure. Dr Samadi also performs other glaucoma surgeries, such as trabeculectomy and tube shunts, when pressure needs to be lowered further. Dr Samadi will explain the options, benefits and risks so you can decide together.

Frequently asked questions

Can glaucoma and cataract be treated together?

Often, yes. MIGS can be performed at the same time as cataract surgery.
